Zephair Flight 1120 (ZA1120) was a scheduled passenger flight from Nexus, the capital city of Ikonia, to Dustgate, principal city of Bastion, that crashed into the Honua Kahiko OCH megastructure on 13 October 2186.

The inexperienced co-pilot, Tukk Konpala, was in control of the aircraft at the time of the accident. Originally an aviator from Sahrland, Konpala was undergoing training with Zephair as part of an enrollment programme. Heavy rain and inclement weather impaired visibility and neccesitated the use of instruments for navigation. He mistakenly believed the aircraft had overflown Kuluust, the turning point to fly west, and began descending towards what he thought was Quinarin Airport in Bastion. He failed to notice that instrument readings indicated he was still 22–27 km (14–17 mi) east of Kuluust. Konpala, upon descending below the low-lying clouds and regaining visual flight conditions, saw the north face of the Honua Kahiko and unsuccessfully tried to gain altitude. The aircraft struck the dome, shearing off both wings and the tail cone. The remaining portion of the fuselage passed through the relatively thin outer shell of the dome at an estimated 350 km/h (220 mph), falling 2,375 metres (7,791 ft) through the interior sky of the dome before impacting the ground.

The flight was carrying 87 passengers and eight crew, including nine primes who had been attending a conference in Nexus. Seven crew members and 79 passengers were killed during the crash, and several more died soon after due to the severity of their injuries. Search and rescue aircraft were unable to observe the area due to the crash site's position inside the dome, but aircraft patrolled the perimeter of the dome in case survivors or inhabitants left the area. Search and rescue efforts were called off after eight days of searching. It was assumed by then that nobody could have survived the crash.

In total, 14 kerbals survived the crash. Seven died of their injuries, one was killed by wild animals, and two were killed by the inhabitants of the OCH. After nearly two months surviving inside the harsh environment of the dome, only four survivors were rescued.

45	26 November	Another group of inhabitants is discovered whilst the survivors collect water. Both parties flee.
47	28 November	Thadox Linwey Goes off alone to attack the inhabitants, seeking revenge for the loss of Sule.
49	30 November	The survivors hear the sounds of violent combat over the wind. Thadox kills two inhabitants before being dismembered.	4
50	1 Deccember	In the face of enormous backlash from citizens of Oltum, apex Hector personally flies to OCHKo and insists that the survivors be extracted.
52	3 December	The Ikonic Air Force makes contact with the survivors. In an extraordinary breach of OCHKo protocol, a message is dropped through the hole in the dome wall and guided to the camp. The survivors are instructed to head for the east wall of the dome where the OCH will be opened for the first time since its construction.
54	5 December	The survivors arrive at the pick-up point and are rescued.

Effect on the Honua Kahiko

More PSALM's were utilized during the cleanup operation than any other time in Ikonic history. Several inhabitants received such a high dosage of amnesiating PSALM's that the sulci in their brains were irreparably smoothed, rendering them permanently lobotomized.

Despite best efforts to conceal and suppress the incident, the inhabitants' culture has been irreparably altered by the visitation form outside the dome.
